Pre-admission assessments
All prospective residents are fully assessed by our Home Manager prior to admission to establish that their needs can be met within our home. This assessment is used as the basis for future care planning which is an ongoing process following admission to Fairmont. Care plans are created by the management team in consultation with the resident, their relatives and other medical professionals.
Care Planning
Each resident’s care plan will include a description of their preferred daily routine, their likes and dislikes in relation to food and any special dietary, health or personal needs. It will also address the issues of dignity, privacy and respect and what this means to the resident in terms of daily behaviour and actions. In addition the care plan includes a risk assessment and management plan. Finally it includes details of residents’ social interests and hobbies and how these are met.
Keyworker scheme
Every resident is allocated a key worker who will become their primary point of contact. The aim is to create a rapport between the resident and staff member so that the resident feels secure that all their needs can be met.
External links
The home maintains good relationships with all local healthcare agencies and social services departments enabling us to access any services that may be required by a resident, for example, chiropody, speech therapy or physiotherapy. Residents are encouraged to remain with their existing GP if at all possible as this maintains continuity of care.
